Due to investment and development in our new Community Response Hubs we are looking to recruit a full time Community Nurse Band 6, Clinical Lead working 37.5 hours per week between the hours of 8am – 8pm.

Based in Ellesmere Port visiting the patients of the Ellesmere Port and Neston areas. Travel is an important part of this role. Access to a car for work purposes is essential.

The post holder will be a clinical resource, providing a visible clinical profile, having advisory, leadership and clinical oversight of the caseload.

The team focus is on prevention of unnecessary acute hospital admission, End of Life Care and Facilitating discharge from hospital.

By working collaboratively with the wider healthcare team we ensure person centred care for those patients whose preferred place of care is at home.

A high level of communication is essential for the role, with Team work at the core of our practice.
