Job Summary This is an exciting opportunity to join our existing team of Clinical Pharmacists working at Scott Arms Medical Centre on behalf of our PCN Health Vision Partnership. The successful candidate will be part of a dynamic and forward‑thinking team employed by Health Vision Partnership working at Scott Arms Medical Centre. Professional leadership will be provided by Scott Arms Medical Centre Clinical Lead Partner. This is a full‑time position of 37.5 hours per week, although alternative working hours may be considered for the right candidate. The closing date is 04 July 2026.

Responsibilities

Improve patients' health outcomes and the efficiency of the practice team by providing direct, accessible and timely medicines expertise, including identifying areas for improvement and initiating change.

Act as the primary contact within the practice for queries relating to medicines management, in particular around individual patient care.

Address immediate and future needs to personalise care planning for patients.

Proactively transfer workload relating to medicines optimisation from other clinical staff to improve patient care and safety.

Demonstrate the effectiveness of the practice and the role through audit and data analysis.

Follow up patients and investigations, particularly blood tests.

Act as a clinical pharmacist supporting and working alongside a team of health‑care practitioners within general practice and a Primary Care Network.

Provide primary support to general practice staff with prescription and medication queries; perform medicines reconciliation on transfer of care; implement safer prescribing systems, providing expertise in clinical medicines advice and addressing public and social care needs of patients in the PCN.

Work as part of a multi‑disciplinary team in a patient‑facing role; take responsibility for medicines optimisation within the PCN and undertake structured clinical medication reviews to proactively manage patients with complex polypharmacy.

Provide clinical leadership on medicines optimisation and quality improvement (QI) and manage aspects of the quality and outcomes framework and enhanced services at practice and PCN level.

Develop and quality‑assure improved, safe and efficient repeat prescribing and medication review processes within the practice, and evolve and implement changes as required.

Conduct focused and structured reviews of repeat prescriptions on an individual basis to improve quality and safety and maximise effectiveness.

Deliver medication review services via clinics in the practice, domiciliary visit and in residential and nursing homes, either face‑to‑face or notes‑based.

Undertake disease‑focused medication reviews for high‑risk groups (elderly, renal/hepatic impairment); ensure compliance with national and local guidelines through pharmacist‑led clinics, telephone and office‑based notes reviews.

Ensure safety and appropriateness for patients taking high‑risk drugs such as DMARDs and anticoagulation.

Provide telephone advice to patients with long‑term conditions on specific medicine‑management issues.

Contribute to the development of practice ICB long‑term conditions pathways to ensure consistency and efficiency of systems.

Provide health promotion, including flu vaccinations.

Provide acute‑illness and pharmacist‑led clinics within clinical scope: initial assessment of common general practice presentations; differential diagnosis and referral; independent diagnosis and management of minor illnesses; follow‑up with GP mentor support; telephone advice for acute illness queries; triage of minor ailments with signposting to non‑GP services.

Review and reconcile medicine changes from secondary care organisations, particularly discharge papers and TTOs; reconciling computer records; ensuring concordance with national and local policies; ensuring optimisation of treatment and reducing inappropriate or wasteful prescribing.

Follow up hospital discharge/outpatient letters, ensure action such as blood tests and medication titration, and inform patients of changes.

Manage practice formularies and IT systems to ensure appropriate selection of medication and maximise prescriber use.

Facilitate development of electronic prescribing and increase its usage.

Maintain accurate patient medication records on the clinical computer systems, including advice and action taken in pharmacist clinics/reviews.

Ensure appropriate governance regarding medicines, leading on responses to drug safety alerts or changes in clinical guidance.

Assist with development of medicines‑management guidance and policies; advise primary healthcare team on safe handling of controlled drugs; identify risks; offer advice on adverse events and patient complaints; conduct audits and monitoring of prescribing; report to GP partners; analyze prescribing data; lead on medicines‑management targets and initiatives.

Provide training in medicines‑related issues to other practice members; develop plans for reception team training; analyze complex information and prepare evidence‑based summaries for clinicians, staff, and patients; prepare resources and information for rational cost‑effective prescribing; present at patient group meetings or other appropriate events; produce patient information leaflets and posters; raise awareness of out‑of‑practice schemes such as minor ailments and self‑care promotion.

Qualifications

Mandatory registration with the General Pharmaceutical Council (GPhC).

Experience

Experience working as a clinical pharmacist.

Involving patients in decisions about prescribed medicines and supporting adherence as per NICE and local best practice guidelines.

In depth therapeutic and clinical knowledge and understanding of evidence‑based healthcare principles.

Appreciation of the nature of GPs and general practices.

Appreciation of primary‑care prescribing concepts, rational prescribing and strategies for improving prescribing.

Variability in skill and competence level appropriate to the role.
