Band 7 Community Adult Haemoglobinopathies Nurse Specialist
We are seeking a highly motivated and experienced Specialist Nurse to join our Haemoglobinopathies Service, providing expert care and support to individuals and families affected by sickle cell disease and thalassaemia.
Working within a well-established multidisciplinary team, you will deliver high-quality, patient-centred care and collaborate with colleagues across the organisation and specialist haemoglobinopathy network to ensure effective care coordination and positive patient outcomes.
As a senior specialist nurse, you will act as a key resource for patients, families and healthcare professionals, providing specialist advice, education and support throughout the patient pathway. You will build strong therapeutic relationships, empowering patients to manage their condition and participate in decisions about their care.
The role includes clinical leadership responsibilities, promoting evidence-based practice, supporting colleagues and contributing to service development, quality improvement and innovation. You will help shape the future delivery of haemoglobinopathy services while ensuring care remains responsive to the needs of patients and families.
This is an excellent opportunity for an experienced nurse to further develop their specialist expertise and leadership skills while making a meaningful difference to the lives of people living with haemoglobinopathies.
In conjunction with the haemoglobinopathies teams across North Central London, the post holder will be responsible for the delivery and ongoing development of a high-quality specialist sickle cell and thalassemia service in the community. The post holder will provide clinical leadership and practice at an advanced level demonstrating a high level of autonomy and clinical decision making in the community.
Implementation of a patient outreach health education group programme, and organisation of patient led education sessions.
Developing close links with multidisciplinary colleagues in primary, secondary, and social care to enhance continuity of care for patients
Enhancing compliance with preventative actions such as vaccinations and prophylactic medications
Provision of health education and guidance on self-management to prevent exacerbation of patient’s condition to reduce incidences of acute episodes requiring admission to secondary care
To enable implementation of individual care plans for patients in all settings
Targeted support to high-intensity users of secondary care including directing patients into existing NCL programmes for frequent ED attenders
Lead by example and act as a positive role model for all staff, taking responsibility for own professional development
