Rosedean House Surgery is seeking an experienced Community Matron to join our primary care team. We are proud to have recently been rated ‘Good’ by the CQC and remain committed to continuous improvement as we further develop and enhance our services.
The Community Matron will support patients of all ages with diverse and often complex health needs across the Liskeard community. They will provide personalised, holistic care for adults with long-term conditions, frailty, and multiple comorbidities — helping patients live well at home, in care homes, or in other community settings. Given that a significant proportion of our population is older, this role will include a focus on supporting frail and elderly patients, while also addressing the broader needs of adults of working age with complex or chronic conditions.

- Reducing avoidable hospital admissions
- Coordinating care across primary, community, and social care services
