Are you a highly motivated, organised person with excellent interpersonal skills? An exciting opportunity has now become available for one suitably experienced Community Outreach Worker to join our friendly team.

The role of Community Outreach Worker is to support the care management process, working to support adults with a variety of needs in order to promote independence, health and wellbeing. The role includes supporting people to improve their quality of life, whilst encouraging the person to develop wider links within the community. The post holder will be responsible for signposting and facilitating involvement with a range of services and agencies via research, advice, information and support in the local/wider community.
