We have an exciting opportunity for a Band 5 enthusiastic, motivated, and compassionate nurse to join our established multidisciplinary Care Community Team to support and deliver a range of quality nursing care to our community patients in their own homes.

Our care community team is based in Tattenhall , covering rural West Cheshire and surrounding geographic area . It’s important that you can drive and have access to your own vehicle for work as travel is an integral part of the role. But of course, mileage expenses are reimbursed in addition to your hourly rate.
As a Community Staff Nurse the successful candidate will undertake duties between the working hours involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.
You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire.
