Dynamic Support Pathway Clinical Lead
Leicester, England, United Kingdom Internship Posted 3 days ago
The Dynamic Support Pathway (DSP) coordinates support for people with a confirmed diagnosis of Learning Disability, Autism, or both who are experiencing a decline in their mental health whilst living in the community. The team’s purpose is to bring involved professionals together, and agree additional support to prevent deterioration, crisis, or hospital admission. The DSP also supports people stepping down from inpatient units, co-ordinating discharge planning and post discharge support.
The pathway applies to both adults and children/young people (CYP), using the same referral process.
The service operates Monday to Friday 9-5.
An exciting opportunity has arisen for a Band 7 Team Lead to provide leadership and management to the DSP Team ensuring that the existing standard operational procedures and relevant local and national policies are implemented to a high standard. The post holder will also hold some operational responsibilities in relation to LDA Discharge Hub Case Managers.
The successful candidate will have experience of working with people with learning disabilities and/or autism and recognise the significance of early interagency collaborative working, and intensive support.
