Rapid Response Support Worker - Drivers

Greater Swansea Area Full-time Posted 3 weeks ago
This isn’t your average care role - Be the reason someone gets to go home!

Full UK Driving Licence Required

As a Rapid Response Support Worker, you’ll step in at critical moments — helping people with complex needs leave hospital safely and rebuild life at home with their families or step in to support stabilising a care package to enable clients to remain in their own home. You’ll bring calm, confidence, and consistency when it matters most.

If you’re experienced, adaptable, and motivated by making a real difference, this role was made for you!

Why You’ll Love This Role

  • See the direct impact of your work — often within days
  • No two assignments are the same
  • Work autonomously while being supported by clinical and care teams
  • Help families through some of their most stressful moments
  • You’ll be trusted to lead, stabilise, and mentor

This is a role for someone who thrives on responsibility and purpose!

You’ll support the set-up and stabilisation of both new and existing complex care packages, including:

  • Supporting safe and timely hospital discharges
  • Providing high-quality personal care and daily living support
  • Creating routines and helping clients regain independence
  • Offering reassurance and continuity to families
  • Mentoring new Support Workers and modelling best practice
  • Liaising clearly and professionally with Care Managers and clinical teams
  • Organising essentials such as medication, supplies, and care equipment

Flexibility That Works Both Ways

  • 162 working hours per month
  • Breaks between assignments - length depends on length of previous assignment
  • Overtime maybe available if required
  • Work live-in placements (sometimes 2+ weeks for continuity)
  • Work live-out shifts (accommodation will be provided if not within daily traveling distance)
  • Travel across the UK to support care packages

This role suits someone who enjoys variety and flexibility.

What We’re Looking For

  • Full UK Driving Licence
  • Proven experience in complex care
  • Confidence working independently
  • Strong communication and organisation skills
  • A calm, professional approach with clear boundaries
  • Hospital discharge experience is desirable
  • Level 2 Health & Social Care qualification or above is desirable

If you’re ready to use your skills where they truly matter, we’d love to hear from you. Apply today.
