Job Summary Lead and develop specialist clinical pharmacy services within the Emergency Admissions unit at Gateshead Health NHS Foundation Trust.
Responsibilities
Provide specialist clinical pharmacy input and maintain high standards of medicines management across the Emergency Admissions unit.
Work closely as part of the MDT, providing prescribing support, following up medicines reconciliation discrepancies, prescribing acute medication, and supporting medication rationalisation.
Provide patient medication counselling and facilitate safe discharge to ensure patient flow.
Collaborate with the urgent and emergency care pharmacist on service provision and development within the Emergency Care Centre.
Support development and updates of clinical guidelines, policies and SOPs.
Provide leadership, clinical supervision and line management to pharmacy colleagues.
Contribute to innovation in urgent and emergency care, seeking opportunities for admission avoidance, medicines optimisation, and improved transfer of care between hospital and community.
Work to deliver a 7‑day clinical pharmacy service, supporting the late‑night, on‑call and weekend rota.
Support the Lead Clinical Pharmacist with medicines management, incident resolution, staff training and integration with the Emergency care senior team.
Engage key stakeholders including ward managers, matrons, service line managers and clinical and safecare leads.
Experience
Clinical pharmacy experience in a hospital setting.
Experience of leadership.
Experience of clinical audit or quality improvement.
Desirable Experience
Experience as a line manager.
Experience of service development or improvement.
Working in the emergency admissions unit.
Knowledge and Skills
In‑depth clinical pharmacy knowledge.
Excellent written and verbal communication skills.
Excellent consultation skills.
Specialist knowledge of pharmacy systems.
Evidence of self‑reflection and continuous professional development.
IT literate.
Ability to prioritise own workload.
Desirable Knowledge and Skills
Evidence of innovative practice.
Qualifications
Masters degree in Pharmacy.
Registered as a pharmacist with the General Pharmaceutical Council (GPhC).
Diploma in Clinical Pharmacy or equivalent experience.
Independent Prescriber.
