Job Overview

You will work as a member of the multi-disciplinary team in providing high quality, evidence-based, holistic care to patients in line with professional codes of conduct and Trust policies. You will access, plan, implement and evaluate packages of care based upon individual service user needs, under the guidance of the Advanced Nurse Practitioner as per NICE guidance. You will also undertake and maintain service users’ Self Directed Support assessments.

As required by your manager, you will have responsibility for the day to day supervision and management of lower banded staff within your professional group and team locality. This will include the management of these staff in line with agreed Trust HR processes.

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 5-day a week day service.

In delivering your duties you are expected to display behaviours that are aligned with the Trust values and in accordance with the NHS Constitution at all times.

Main duties of the job

Key Responsibilities

You will deliver high-quality, person-centred care, supporting service users through assessment, care planning, and review. Interventions must be evidence-based, recovery-focused, and aligned with Trust and NICE guidance.

You will maintain accurate, timely clinical records in line with professional standards. Duties include undertaking assessments, completing risk evaluations, and implementing management plans to ensure safety for individuals and environments.

Working within a multidisciplinary team, you will contribute to meetings, share clinical insight, and support coordinated care. You may act as a care coordinator or key worker, managing a caseload and liaising with carers and professionals.

You will carry out physical health checks and respond to findings appropriately. Registered nurses will administer medication safely, monitor effectiveness and side effects, and promote adherence through education.

You will provide psycho-social interventions, practical support, and education to promote recovery, independence, and relapse prevention.

You will also support service development, meet performance targets, contribute to policy implementation, deliver training where appropriate, and maintain ongoing professional development while upholding Trust values.
