Job Description Summary

  • Work autonomously, taking responsibility for care quality, clinical activity, and management within the Community Nursing Hub. Lead assessment, coordination, and development of specialist interventions for patients with complex health or social needs. Oversee triage and caseload management using advanced clinical judgement, working closely with the MDT to address physical, mental, and social needs.
  • Provide managerial and clinical supervision to registered, unregistered staff, and students, leading supervision sessions across the Hub. Maintain effective communication with colleagues, external agencies, and stakeholders to support optimal health, selfmanagement, and appropriate care delegation.
  • Implement evidencebased care plans, deliver direct patient care, and act as a clinical role model. Communicate complex patient information to the MDT and lead staff and team management. Manage daytoday Hub operations and deputise for the Community Service Matron.
  • Ensure delivery of a clinically effective service aligned with Trust policies. Hold professional and managerial accountability for staff, budgets, quality, and standards, strengthening systems such as supervision, development planning, audit, and policy compliance. Promote excellence in nursing practice and patient, carer, and family involvement. Complete required leadership training. Must hold a Specialist Practitioner Qualification in District Nursing.
