What You'll Be Doing

  • Promote and embed strength-based, outcome-focused social work practice across Adult Social Care pathways.
  • Support individuals to maximise their independence, wellbeing, and personal choice.
  • Drive the principles of Making It Real, Making Safeguarding Personal, and asset-based community approaches.
  • Manage and support complex cases involving vulnerability, safeguarding concerns, risk management, and crisis intervention.
  • Undertake and oversee assessments, reviews, and support planning in line with legislative requirements.
  • Provide professional guidance, coaching, supervision, and mentoring to Social Care Assessors and colleagues.
  • Quality assure practice and contribute to service improvement initiatives.
  • Work collaboratively with internal teams, health partners, providers, families, and local communities.
  • Support the development of resilient communities that enable people to stay connected, safe, and well.

About You

We are looking for an experienced, motivated, and knowledgeable Social Work professional who is passionate about delivering high-quality, person-centred services and achieving positive outcomes for adults and their families.

You Will Have

  • A recognised Social Work qualification (BSc Social Work, DipSW, CQSW, or equivalent recognised by Social Work England) and current Social Work England registration.
  • Significant post-qualification experience within Adult Social Care, Health, or a related field, including managing complex casework involving safeguarding, risk, vulnerability, and crisis intervention.
  • Experience of coaching, mentoring, supervising, and developing practitioners, with the ability to provide professional guidance, quality assurance, and support effective decision-making.
  • Strong knowledge and practical application of key legislation, policy, and best practice, including the Care Act 2014, Mental Capacity Act/DoLS, Making Safeguarding Personal, personalisation, Continuing Healthcare, rehabilitation and reablement, transitions, Transforming Care, and preventative approaches.
  • The ability to work collaboratively across multi-disciplinary teams and Adult Social Care pathways, building effective relationships with customers, families, colleagues, and partner organisations.
  • Excellent analytical, organisational, and communication skills, with the ability to prioritise competing demands, meet deadlines, and deliver high-quality outcomes.
  • The ability to undertake AMHP and/or Best Interests Assessor (BIA) assessments, or a willingness to develop these skills where applicable.
  • A flexible approach and the ability to visit customers and families in their own homes as required.
