Specialist Occupational Therapist

We have a exciting opportunity for a Band 6 Occupational Therapist post within the Rural Alliance Care Community Team based in Chowley Oak as part of an integrated care team.
You will be part of an Integrated Care Community Team providing Therapy for patients in their own home. This will involve working with patients to remain in their own home in community and support with rehabilitation of patients.
The team works across rural Cheshire working alongside 5 GP practices to deliver care to the practice population
You could already be a Band 6 wishing for a change, or currently an experienced Band 5 who would like to take the next step in your career and practice in a community setting.
This post is part time 30 hours per week.
To work autonomously as a specialist therapy practitioner within an integrated Care Community Team to deliver a high-quality Occupational Therapy service through assessment and treatment of patients referred with complex rehabilitation needs
To work as a member of a multi-disciplinary team and communicate information to those agencies (either health or social background) involved, to meet the needs of the patient & plan effective care.
To participate in departmental In-service Training and Journal Club activities to further consolidate knowledge base and specialist clinical skills. Participate in the CPD programme to share these skills with therapy and nursing colleagues.
To provide supervision, teaching and support to senior and junior staff, assistant staff and act as a clinical educator for undergraduate students.
