Job Overview
CNWL are looking for a dedicated and enthusiastic Speciality for a new Speciality Doctor post within the Kensington & Chelsea Community Hubs with a specific focus on difficult to reach patients. It will be part of Assertive Outreach Pathway (AO pathway) which will be fully integrated within the Community Hubs.
Main duties of the job
The post holder will be expected to provide medical input for difficult to engage patients under care of both North and South MH Hubs under the supervision of the Consultant Psychiatrist.
This post is part of a fully integrated multi- disciplinary teams in K&C MH Hubs and will have access to all additional services available in the Hubs (OT, Community Pharmacist, psychology input, depot and clozapine clinics, admins support). Designated software (Anathem) will be available to reduce administrative workload.

- St Charles Hospital with 8 inpatient wards; two adult PICUs, 4 adult acute wards and two Older Adult wards, HBPoS
- Community team bases for adult Mental Health Hubs at Hathaway House (North MH Hub) and South Kensington and Chelsea Mental Health Centre next to Chelsea and Westminster Hospital (South MH Hub)
- Chelsea and Westminster Psych Liaison service, health psychology based at South Kensington and Chelsea Mental Health Centre
- Older Adult services and Memory Clinic
- Mental Health Crisis Assessment Service (MHCAS) at St Charles Hospital
- KCW Early Intervention Service
- Home Treatment Team (HTT)
- Mental Health Act Team (AMHP Team)
- Our Psychology and Psychotherapy Services are embedded in the Mental Health Hubs and acute service including Arts and Music Psychotherapy, and other specialist family and individual psychotherapies
- Re-FOCUS Team (community focus service) at Hathaway House
- Provide high-quality psychiatric assessment, diagnosis, treatment planning, and ongoing medical care for adults with severe and enduring mental illness, particularly those who are disengaged, have additional comorbidity, high risk, or non-adherent with treatment.
- Manage a defined medical caseload, supervised by consultant psychiatrists (RC) working within K&C Community MH Hubs, within the Assertive Outreach Pathway, and working closely with the multidisciplinary team in each Hub.
- Undertake regular clinical reviews (clinic reviews and home visits), including mental state examinations, risk assessment, physical health monitoring, insight improvement and review of treatment response, ensuring care is evidence-based, and recovery-focused.
- Provide middle-grade medical leadership within the Assertive Outreach pathway, contributing to MDT meetings, huddles, and zoning discussions, and offering clinical oversight on risk management and medication optimisation.
- The formulation of specialist opinion and writing of psychiatric reports under the supervision of the Consultant Psychiatrist
- Joint working with external stakeholders when patients’ clinical needs require (Drug and Alcohol Services, Probation Services, Police Liaison, Acute MH pathway and Acute Physical Care Services, Re-Focus team) including undertaking the relevant referrals to other disciplines and services both within the multi-disciplinary team as well as to services and resources external to the service.
- Observation of Trust, K&C MHS and local policies and procedures; including those related to medicines management and physical healthcare.
- Interviewing relatives and informal carers of patients as required.
- The post holder will be expected to provide support, teaching and supervision for MDT staff
Qualifications
Essential criteria
- MBBS (or equivalent overseas)
- Full GMC Registration with a license to practice
- Attended an accredited MRCPsych training course (or equivalent).
- MRCPsych Part 1 or Part 2 (or equivalent). Absence would not preclude an appointment.
- Section 12 approved

- Completed at least 3 years full time post graduate training (or its equivalent on a part time or flexible basis) at least two of which will be in a specialty training programme in a relevant specialty or a fixed term trainee or have equivalent experience and competences.
- Previous Experience in community psychiatry

- Able to communicate clearly both in written and spoken English
- Demonstrates sound knowledge and skill base with the ability to prioritise clinical need.
- Demonstrates ability in basic skills i.e. history taking, examination (mental & physical) and the utilisation of external sources of information in order to develop care and treatment plans to meet patient needs.
- Capacity to utilise a range of approaches to solving problems and making decisions
- Demonstrates understanding of the principles of audit and research within psychiatry and their application to clinical practice
- Able to work cooperatively as part of a team and build and maintain successful working relationships with staff in a variety of different roles.
- Ability to recognise and manage own stress utilising the resources available
- Able to work in an efficient, systematic and flexible manner
- Able to deal with difficult situations and to address them in a responsible and professional manner
- Understanding of the complex nature of patient confidentiality
- Able to treat service users with respect and dignity at all times, adopting a culturally sensitive approach, which considers the needs of the whole person
- Able to build constructive relationships with warmth and empathy, using good communication skills
- Understanding and experience of the use of Rapid Tranquilisation and the management of Psychiatric emergencies
