- An exciting opportunity has arisen for an experienced and motivated Occupational Therapist to join our Therapy Service as Team Lead for Frailty Medicine.
- This key leadership role sits within a well-established multidisciplinary team providing therapy services across our Frailty Assessment Unit and short stay frailty ward. The successful candidate will provide clinical and professional leadership to a team of Occupational Therapists, Physiotherapists and Therapy Support Workers, ensuring the delivery of high-quality, patient-centred care for older adults with complex needs.
- Working closely with colleagues across the multidisciplinary team, you will support admission avoidance, timely discharge planning and patient flow through acute frailty pathways. You will also contribute to service development, workforce support and the delivery of operational and quality objectives.
- We are seeking an individual with experience in frailty, acute medicine or older people's services who can demonstrate strong leadership skills, sound clinical reasoning and a commitment to continuous improvement.
