An exciting position has arisen for a Band 5 Community Nurse to join our friendly and supportive team. You will look after patients in their own homes, delivering high-quality clinical care and supporting them to live independently. Main duties of the job

As a Band 5 Community Nurse, your primary responsibility will be to assess, plan, and deliver high-quality, evidence-based nursing care directly to patients within their own homes. Managing a daily clinical caseload safely with minimal direct supervision, you will confidently exercise clinical decision-making. Your routine will encompass a sophisticated clinical skillset, including wound management, catheter care, gastrostomy and enteral feed management, and the administration of medications via IV and syringe drivers.

In this role, you will provide compassionate, dignified end-of-life care, offering vital emotional and practical support to patients and their families during sensitive times. Acting as a key point of contact, you will collaborate closely with GPs, social services, and wider multidisciplinary healthcare teams to ensure seamless, integrated patient care. Additionally, you will maintain precise documentation for every visit to uphold the highest standards of safety and continuity of care.
