Job summary An exciting opportunity has arisen for a Band 6 Community Mental Health Nurse Specialist to join the Older Adult Community Mental Health and Dementia Nursing Team. You will work as a Nurse Practitioner within a multidisciplinary team, providing assessment, treatment, and support for individuals experiencing mental health difficulties, including those undergoing assessment and diagnosis for dementia. Main duties of the job We are seeking a dynamic and experienced Mental Health Practitioner who is adaptable, motivated, and able to work both independently and as part of a multidisciplinary team. The successful candidate will have experience in undertaking comprehensive assessments, including dementia reviews, physical healthcare assessments, health promotion, social inclusion, and recovery-focused interventions. The post holder will be responsible for supporting, coordinating, and reviewing the care needs of individuals with mental health conditions and dementia within the Older Adult Community Mental Health Team. A key aspect of the role will involve supporting patients in their own homes, working closely with families and carers, facilitating hospital discharge, and collaborating effectively with a range of partner agencies and healthcare services to ensure seamless, high-quality care. You will have experience of multi-agency working, be able to prioritise a demanding workload effectively, and demonstrate strong problem-solving skills and the ability to manage the challenges of day-to-day clinical practice.
