You’ll support people in their own homes through skilled nursing care, helping them maintain their health, independence and well being.
As part of a supportive District Nursing team, you will deliver high-quality care to people with a wide range of health needs. You will work closely with patients, families, carers and partner organisations to ensure care is safe, effective and tailored to individual needs. What You’ll Do
- Carry out holistic assessments and develop person-centred care plans
- Deliver nursing care and treatment safely within your scope of practice
- Work with multidisciplinary colleagues to coordinate and improve patient care
- Maintain accurate patient records and report clinical concerns appropriately
- Support patients and carers with information, guidance and health education
- Registered Nurse qualification with current professional registration
- Understanding of community nursing and evidence-based practice
- Experience working in a healthcare environment and as part of a team
- Strong communication skills, including discussing sensitive information
- Basic IT skills and a commitment to learning and professional development
- Ability to travel between patient visits and work flexibly to meet service needs
