Job summary
Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Nursing and AHP vacancies are also open to Student Nurses and Newly Qualified Nurses/AHPs.
The Home Treatment Service (HTS) is part of the Urgent Care Service, providing hospital-level assessment and treatment in patients' usual place of residence. Supporting people with frailty, long-term conditions and acute deterioration, the service helps avoid unnecessary hospital admissions and enables timely discharge from hospital.
As a Band 7 Urgent Care Practitioner, you will deliver advanced clinical assessment and treatment through planned and urgent home visits, working across a 7-day service. Using advanced history-taking, physical examination, clinical reasoning and risk assessment skills, you will autonomously assess, plan, implement and evaluate care for patients with complex and changing needs.
Working closely with acute trusts, primary care, ambulance services and community partners, you will coordinate person-centred care, make timely clinical decisions and contribute to multidisciplinary planning to achieve the best outcomes for patients and carers.
Main duties of the job
We are seeking an experienced and motivated clinician who can combine expert clinical practice with leadership, education and service development. You will provide senior clinical support to colleagues, supervise and develop staff and students, and act as a role model for evidence-based practice and professional standards.
Alongside delivering high-quality care, you will contribute to governance, audit, quality improvement and pathway development, helping to shape and improve urgent community services. You will be confident managing a dynamic caseload, prioritising clinical need, responding to changing demand and supporting operational service delivery.

Qualifications, training and experience
Essential
Professional registration: current registration with the NMC, HCPC or GPhC in a profession relevant to the role. Substantial post-registration experience in urgent, acute or community care, including management of complex caseloads Evidence of enhanced assessment, clinical decision-making and physical examination skills relevant to the service Experience of supervising, teaching, mentoring or assessing others and contributing to team or service development A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non-EU country) and the ability to travel independently are essential for this role
Desirable
Non-medical prescribing, practice assessor or mentorship qualification, management or supervisory development, and progress towards Master's-level study. Knowledge and skills
