Grade 11 – Entry Social Worker grade and registration with Social Care Wales

Grade 12 – Experienced Social Worker grade ~ minimum of 3 years post qualifying experience and registration with Social Care Wales.

Rhondda Cynon Taf Council is pleased to offer a Transition Social Work position in the Learning Disability West Team in Social Care Services.

We are looking to appoint a motivated and enthusiastic Full-Time Transition Social Worker to join the Learning Disability Team West in Rhondda Cynon Taff, based at Ty Elai, Williamstown, Rhondda.

The Team provides support to adults with a recognised learning disability who live in the area of Rhondda Cynon Taff. The teams work to a preventative, strength-based person-centred model under the principles of the Social Services and Wellbeing Act 2014. The Teams have an emphasis on multi-agency working and you will be expected to work closely with colleagues from Health and a number of other agencies. We also work within the principles of the Mental Capacity Act 2005, often practicing within the remit of Best Interest for individuals who lack the mental capacity to make decisions for themselves, which may involve making applications to the Court of Protection.

As a Transition Social Worker within the team, you will play a key role in supporting young people and their families through the transition from children’s to adult services. You will work closely with the Disabled Children’s Team, health professionals, education providers, families and carers to ensure young people experience a smooth and well-planned transition into adulthood. You will complete strengths-based assessments, support planning, reviews and safeguarding work, with a strong focus on promoting independence, choice and positive outcomes

It would be advantageous for candidates to be up to date with emerging issues and have experience of, or an interest in working within this field. Successful candidates will bring strong assessment, communication, and planning skills to ensure positive outcomes for the individuals with whom we work. Along with demonstrating an understanding of risk management and safeguarding, protecting children and vulnerable adults is a core responsibility of all staff appointed by the Council.

Team members work a combination of Agile Working and working at the office base in Ty Elai, Williamstown, Rhondda.

The successful candidate will have regular 1:1 supervision as well as peer supervision. All our practitioners have the chance to influence the development of our work and are supported by a strong, experienced management team both at strategic and operational levels.

We recognise that social work is professionally and personally challenging and demands considerable levels of skill, commitment, and enthusiasm. We offer a dedicated in-house Learning and Development Centre which actively supports practitioners at each level to maintain their skills and Continuous Professional Development.

You must possess a professional recognised Social Work qualification and be expected to be registered and maintain your registration as a 'Social Worker' with Social Care Wales on appointment.

Those shortlisted for interview will be asked to confirm if they are applying for a Social Worker (GR11) or Experienced Social Worker (GR12) post prior to interview. Those applying for an Experienced Social Worker position will need to be prepared to provide evidence at interview of their registration with Social Care Wales and 3 years of post-qualifying experience.
